Must have served aboard:

• USS Frank E. Evans (any years), 
• Served at the time of the collision on June 3, 1969 on any of the below ships:
  • HMAS Melbourne R21
  • USS Kearsarge CV 33
  • Any Desron 23 ship
    • USS James E Eyes DD787
    • USS Everrett F Larson DD830
    • USS Walke DD723
    • USS Schofield DEG3
    • USS Bronstein FF1037
  • USS Tawasa ATF92
  • HMNZS Blackpool F77
  • HMS Cleopatra F28

Payment plan options available:

• 1 time full payment of $199
• 4 quarterly payments of $49.75
• 12 monthly payments of $16.58

Once the full $199 is paid, all payments will be complete and the member will have an unlimited lifetime membership to the USS Frank E. Evans Association.
